An annual pre-participation physical and copy of health insurance card needs to be completed ASAP and returned to leadership for each registered scout and adult by June 10th. Mrs Whitman will be collecting.

• For resident campers, choose form for events 72 hours or more

• Fill out parts A, B1, B2, and C (part C must be signed by physician)

• Print and submit front and back copy of health insurance form
